What kind of medical Certificate do I need?

For agonistic activity it is mandatory to present an agonistic medical certificate in order to participate to any Federal Championship.

For non-agonistic activity it is mandatory to present a non-agonistic certificate in order to enrol to our courses.

 

How do I get my agonistic certificate?

Every member of an agonistic team will be informed by their coaches at Cus Pisa.

 

How do I get my non-agonistic certificate?

The certificate can be released by a family doctor, regularly inscribed to the National Sanitary System. The certificate must be signed and stamped in its original version.

The certificate can be released by a specialist in Sport Medicine, inscribed to FMSI or by another recognized structure. The document must be signed and stamped in its original version.

Cus Pisa has an agreement with ‘Poliambulatorio Medicina a Misura d’Uomo’ (via di Gello 17c/o Centro Servizi ‘Il Parlascio’ scala B, 1st floor). To get an appointment you can call the number 050-560469, send a Whatsapp message to 331-1913772, or write to the Facebook page of ‘PoliambulatorioMedicina a Misurad’Uomo’.

In order to get you certificate according to the agreement, you need to bring you application form. You can ask for it  to the Cus Pisa Front-Office. You will also need: identity card, social security card, documents attesting previous pathologies.

Costs:

30 euros: medical visit with Basal ECG (Electrocardiogram)

40 euros: Basal ECG under physical effort

55 euros:medical visit plus with Basal ECG under physical effort and BIA (Bioelectrical impedance analysis).

You can pay directly in the medical structure.

 

Documents you need to enrol a course

- Fill the application form you will get from the Front-Office (via Federico Chiarugi 5)in the opening hours, or find the pdf version down this page

Erasmus need to provide: identity card / passport, document attesting their inscription to/ position at the University of Pisa with student identification number, valid medical certificate.

 

Refunds:

Refunds are envisaged only in case of unactive courses and have to be required within the current year (December).
